1) Primary Care Physician and Specialist Visits: (For

2) Urgent Care: Periodic availability through
the Superior Walk-In Clinic. Call MCAC (226-4400) or the Superior
Walk-In Clinic (226-2233) for current availability.
3) Out-patient Radiology: Limited X-rays, mammography,
and ultrasounds through Marquette Radiology Associates, and the MGHS Radiology Department.
4) Laboratory: 10
specific lab panels donated by the Marquette General Hospital
and Bell Memorial Hospital labs (For specific labs click here)
5) RX Coverage: Please ask the prescribing
physician for samples of any medication. If no samples are available
through the physician, please call MCAC to find out if your
medication is available free through manufacturer's sample programs,
or for a $4 co-pay limit at any Marquette area pharmacy (MCAC
will pay up to $1500/year for included generic medication, all
Marquette County pharmacies participate).
6) Dental Care:
(Services include emergency care, cleanings, and limited lab
work.)
The patient is responsible for paying for all services
not donated or provided on a volunteer basis through MAP. Coverage

How do I get My Medications?
1) Does your doctor have samples?
As soon as you get any prescription from the doctor, ask if
they have sample that can be given to you. This is the cheapest
and easiest way to secure your medication.
2) Can you get your medication through samples
and PAP forms?
If your doctor doesn’t have samples available, call MCAC
to see if we have them. We will also let you know if we can
get a 3 month supply of medication for free through the manufacturer
by filling out PAP forms.
3) Should you go to the Pharmacy?
If there are no samples available, and there are no PAP forms
we may ask you to purchase medications at any local Pharmacy
for a $4.00 co-pay. MCAC will pay a maximum
of $1,500 per year for medications.
4) If the answer is “No” to these
four questions, the prescription is unavailable through MAP.
Disenrollment
and Reenrollment
Medical Care Access Coalition encourages all members to provide
needed information in a timely fashion, to understand the benefits
guidelines for the Medical Access Program, and to cooperate with
the volunteer medical staff. As part of your entry into the MAP,
you will need to understand and sign a Patient
Responsibilities Form which you will be expected to adhere
to. If you have been disenrolled from the Medical Access Program,
you may be able to re-enroll. Please call MCAC at 906.226.4400
for reenrollment information.
• Patients are not eligible to reapply for services for
a six-month period following the third missed appointment.
• A patient who does not file a Medicaid application at
DHS within 3 business days of applying for MCAC and/or follow
through with the Medicaid process within 14 calendar days of Medicaid
application, will be disenrolled from the program until the completion
of the Medicaid process at the Family Independence Agency.
• MCAC reserves the right to disenroll a patient for any
reason. A review of disenrollment may be performed by the Executive
Director and the Executive Committee.
