4. Residency: A patient must be a Marquette County resident and provide proof of residency.

· A patient must be a Marquette County resident and provide proof of residency. Proof of residency includes but is not limited to: drivers license, utility bill, telephone book listing, tax statement, or other documents that evidence residency.

· If a person is a resident of a licensed public or private institution, they are not eligible for MCAC. This includes but is not limited to residency in: a nursing home, jail, prison, halfway house, mental health or substance abuse treatment center.
