Access to Care Community Coalition


With support from the W.K. Kellogg Foundation, the Center facilitates quarterly meetings for Michigan communities working on expanding access to health care initiatives targeted at the uninsured. This networking opportunity allows participants to provide program updates and exchange best practice information. It also helps build community-to-community collaboration in the problem solving, developing new programs, and pursuing funding. Additionally, the group is updated on federal and state access to care developments and provided with the opportunity to meet with public policymakers.

2004 Member Profile

As was agreed at the ACCC Colloquium on February 24, all participants in the Colloquium will fill out an information sheet with basic information about their community health initiative. This information is very important for the message about the ACCC that we will be sending to state policy makers, and for participation in the tobacco tax support campaign. The survey form below may be: 1) printed out, completed, and mailed or faxed to Ann McMillan at CACH-MPHI (see directions on the form); or 2) downloaded, completed, and emailed to Ann McMillan at CACH-MPHI. Please take a few minutes to do this.
